Moving into a treatment home is a significant step. Our admissions process is designed to be transparent, collaborative, and respectful of the person at its center.
Four steps from first call to welcome home
The prospective resident takes part in every step. Before admission, we obtain informed consent for services from the individual or their legal representative, as Oregon rules require.
Referral
A case manager, county mental health program, hospital, care organization, or family member sends a referral packet. We contact the referring agency within 48 hours.
Screening visit
Within 14 days, the prospective resident is invited to a face-to-face screening interview at the home. They receive information about the home and its services, can ask questions, and may meet a prospective roommate. The Program Administrator shares a decision within 72 hours of the screening.
Planning
We confirm funding, complete consents and releases, review the residency agreement and resident rights together, and plan a move-in date with the care team.
Welcome
After a supported arrival and a health screening, we complete an assessment and develop the resident's service plan together with them and their care team.
Referrals welcome from
- County community mental health programs (CMHPs) in Multnomah, Washington, and Clackamas counties and beyond
- Coordinated care organizations (CCOs)
- Psychiatric hospitals and the Oregon State Hospital
- Residential treatment facilities and adult foster homes
- Outpatient providers and case managers
- Families and individuals, working with a care team
A good fit may include
- Adults 18 and older
- A diagnosed serious mental illness
- Medically stable enough to live in a community setting
- Needing 24-hour support to live safely and work toward goals
- Willing to take part in screening and a service plan
Final admission criteria will follow Oregon rules and be shared with referral partners before we open. Every referral is reviewed individually.
Clarity about cost, before anything is signed
In Oregon, residential mental health services are often funded through a combination of sources, which may include the Oregon Health Plan through a coordinated care organization, county programs, or private pay.
We are finalizing our payer arrangements now and will publish the options we accept once they are confirmed. Until then, we are glad to talk through your situation. Asking a question never costs anything.
What to have ready
Oregon rules ask for these in a referral packet:
- Written documentation of the mental health diagnosis
- Background and history of services
- Medical information and current medications
- Legal documents, such as guardianship, conservatorship, commitment status, or advance directives
- The most recent mental health treatment plan
- Information about the person's ability to evacuate in an emergency
