Structure that supports, never smothers.

Twenty-four hour residential mental health support in a home setting. Here is who we are building Belmont for, and what care will look like once we open.

Who we'll serve

Adults who need more support than living alone allows

Belmont is being designed for adults 18 and older living with a serious mental illness who are medically stable and ready to live in a community setting with daily support. That may include people who are:

  • Stepping down from a psychiatric hospital or the Oregon State Hospital
  • Moving from a larger residential treatment facility toward more independence
  • Needing more structure after trying to live on their own
  • Living with schizophrenia, schizoaffective disorder, bipolar disorder, major depression, or a related condition

When another setting may fit better

We will always be honest if Belmont is not the right level of care. A different setting may serve someone better if they:

  • Need ongoing skilled nursing or hospital-level medical care
  • Need medically supervised withdrawal from alcohol or drugs
  • Need a locked or secure setting to stay safe

Services

What residents can expect

Every resident will have an individualized service plan, written with them and reviewed regularly. Planned services include:

i.

Daily living skills

Hands-on support with cooking, cleaning, laundry, personal care, budgeting, and keeping a schedule.

ii.

Medication support

Residents who can safely manage their own medications do so. Others receive help from staff trained by a licensed medical professional. All medications are kept in locked storage.

iii.

Symptom management

Recognizing early warning signs, practicing coping skills, and following a personal wellness plan.

iv.

Social and life skills

Communication, relationships, problem solving, and handling everyday stress in a supportive setting.

v.

Community connection

Getting to appointments, using transit, and joining social, educational, volunteer, and work activities.

vi.

Health and wellness

Support with medical, dental, and vision care, nutrition, sleep, and physical activity.

A day at Belmont

A predictable rhythm, with room for choice

Routine is one of the kindest things a home can offer. Here is a sample weekday. Each resident's day will reflect their own plan, appointments, and interests.

Care coordination

Part of the team

Psychiatric care and therapy will be provided by each resident's own community providers. Belmont's role is to be the steady, daily link between them.

  • Regular communication with prescribers, therapists, case managers, and primary care, with written consent
  • Daily observations that help providers make good decisions
  • Help arranging and preparing for appointments
  • Collaboration with county programs and care organizations
Transition planning

The next step, from day one

Belmont is a place to grow stronger, not a place to get stuck. Every service plan will include what comes next, whether that is supported housing, living with family, or a home of one's own.

  • Independence goals in every plan
  • Gradual practice with the skills the next setting requires
  • Coordinated discharge with the resident's care team
  • Ongoing supports in place before moving out
Resident rights

Residents are the authors of their own lives

Oregon law protects the rights of people living in residential treatment homes, and we will honor them in letter and in spirit. Every resident receives a written copy of their rights and our grievance process at move-in.

  • The same civil and human rights as every other Oregonian
  • Privacy in their own room, with a door they can lock
  • Visitors of their choosing at any time
  • Confidential mail, phone calls, and private visits
  • Control of their own schedule and access to food at any time
  • To decorate their room and choose a roommate if sharing
  • To take part in the community, including work, recreation, and worship
  • To take part in creating their own service plan
  • To raise a concern without fear of retaliation
